方法:本研究於2024年5月至12月在一家員工超過500人的職場,以「動員-評估-規劃-執行-追蹤(MAP-IT)」五步驟架構設計戒菸介入。組建跨專業戒菸團隊;調查職場吸菸盛行率、現行菸害防制政策與員工參與意願;制定戒菸服務流程、工作職責、介入策略(5A’s、5R’s及SDM)與獎勵機制;於職場設置戒菸門診與衛教空間,提供戒菸諮詢和輔助工具;以尼古丁成癮程度、一氧化碳濃度、滿意度、戒菸自我效能及戒菸率評估成效。
結果:結果共計38位吸菸員工參加,出席率73.7%,全勤參加率36.8%。參加者戒菸自我效能經介入後均提升且達統計上顯著差異(p<0.05);戒菸成功率三個月為44.7%、六個月為42.1%。
結論:運用MAP-IT架構的職場戒菸服務在職場取得了正面成效,具有擴大應用的潛力。

Applying the MAP-IT Framework to Enhance theEffectiveness of Workplace Smoking Cessation Services:A Practical Experience

Purpose: Smoking has adverse effects on both individual health and workplace productivity,increasing the risk of various diseases and contributing to reduced productivity. Quitting smokingdelivers substantial health benefits for individuals of all ages. The workplace serves as an idealsetting for planning and implementing smoking cessation programs, which has been shown toimprove employee health, reduce the disease burden caused by smoking, lower absenteeism rates,enhance productivity, and decrease medical costs. The study applies the MAP-IT framework toprovide smoking cessation services for employees and examines changes in smokers' self-efficacyand cessation success rates.
Methods: The study was conducted from May to December 2024 in a workplace with over 500employees, adopting the five-step MAP-IT framework—Mobilize, Assess, Plan, Implement, andTrack—to design a smoking cessation intervention. A multidisciplinary smoking cessation team wasestablished, and the prevalence of smoking in the workplace, existing tobacco control policies, andemployees' willingness to participate were assessed through surveys. Based on the findings, the teamdeveloped the service process, defined work responsibilities, and designed intervention strategiesincorporating the 5A’s, 5R’s, and Shared Decision-Making models, along with incentivemechanisms. A workplace smoking cessation clinic and health education corner were thenestablished to provide counseling and supportive tools. Intervention effectiveness was assessedusing relevant indicators such as nicotine dependence level, carbon monoxide concentration,satisfaction, smoking cessation self-efficacy, and cessation rates.
Results: A total of 38 smoking employees participated in the program, with an attendance rate of73.7% and a full participation rate of 36.8%. Participants’ smoking cessation self-efficacy improvedsignificantly after the intervention (p < 0.05). The smoking cessation success rate was 44.7% at threemonths and 42.1% at six months.
Conclusion: The workplace smoking cessation intervention implemented using the MAP-ITframework achieved positive outcomes, showing potential for broader application in otherorganizations.
